Yes, ALL companies with the authority to write workers’ compensation policies must complete this agreement to include the company’s NAIC/WI/IWG number, contact name, phone number and email address. This agreement must be completed and emailed to Sifsurcharge@labor.mo.gov. Upon receiving this completed agreement, the Division then can supply your company contact with the unique Access Code & ID Number that is assigned to your company.
